Any person who knowingly and with intent to defraud any insurance company or person files an application containing any materially false information or conceals, for the purpose of misleading, information concerning any fact material thereto commits a fraudulent insurance act, which is a crime under applicable law. The applicants and indemnitors certify the truth of all statements in the application and authorize the company to verify this information and to obtain additional information from any source including obtaining a credit report.

Each indemnitor releases such third parties from liability resulting from the provision of such information. Submission of this form is equivalent to a signature.

Our team will run a soft inquiry (also known as a soft credit pull) as part of our underwriting process. In order to do this, we do require the social security numbers of each of the indemnitors and their spouses.

Note: If you choose not provide them via this online application, we will need to reach out to you directly to obtain them. This will cause a delay in our approval process, so please be sure to respond to us as soon as possible when we contact you for the information.
